Number of beds: 52 beds, all private bed.    Primary patient population/diagnoses: - Age groups served: - Adult and geriatric patients - Pediatric patients ≥ 15 years of age and ≥ 40 kg - Medical diagnoses - Cardiac diagnoses - Stable patients with acute coronary syndrome - Stable patients with decompensated CHF - Endocrine disorders, including, but not limited to, diabetes - End-of-life care when transferring to home or an inpatient hospice setting is not realistic based on life expectancy - Infection diseases - GI disorders, including, but not limited to, Crone’s disease, obstruction - Neurological disorders, acute and chronic, including, but not limited to: - Brain injury - Multiple sclerosis - Seizure monitoring with continuous EEG monitoring - Spinal cord injury - Oncology diagnoses, including, but not limited to: - Conditions related to the cancer, e.g. chemotherapy, neutropenia, symptom management related to treatment, crisis management and/or pain control. - Specific care related to oncologic conditions, administration of chemotherapy, and surgery. - Pulmonary, including, but not limited to, COPD, stable ventilator management, respiratory isolation - Psychiatric patients requiring acute medical intervention - Overflow surgical patients as per surgical scope of service

Legacy Meridian Park Medical Center is the South Metro area’s full-service medical center, with the friendly feel of a community hospital and the skill and services of a much larger medical center. Among our specialties: stroke, chest pain, minimally invasive and robotic surgery, cancer care and orthopedic and total joint surgery. Plus a birth center and breast health center.   LPNs take care of patients that have long term treatment needs. Often, these patients do not have a safe discharge plan to complete the treatment elsewhere and therefore continue to have a bed in the hospital. LPNs could have the opportunity to have hands on experience with wound care and IV antibiotics, among other long term care treatment. LPNs may often work beside acute care RNs but not directly in acute patient care areas.  They work in collaboration with a C.N.A., charge RN, unit coordinator and physician. LPN staff with a variety of experience and backgrounds work on this unit. We strive to support learning and continuted education with all staff. Complex discharge units have medically stable but complex patients who are pending a safe discharge plan.    The LPN must think critically, be able to problem solve and be ready to learn. Excellent communication with teammates and patients is essential. The ability to maintain compassion for this complex patient population is also essential.   Staff on this unit value teamwork. Our physician partners work closely with this team to support in providing the best care posible for our patients.
