Established in 1987, CARES Northwest is one of nation’s oldest children’s advocacy centers and the largest in Oregon. Our program is a unique collaboration of four health systems – Randall Children’s Hospital at Legacy Emanuel, OHSU’s Doernbecher Children's Hospital, Kaiser Permanente, and Providence Children’s Health – and child abuse multidisciplinary partners in Multnomah and Washington counties. The Family Navigator – Culturally Responsive (FN-CR) is a Social Worker with comprehensive knowledge of trauma treatment modalities, crisis intervention, and case management, and specializes in working with patients and caregivers from our Latinx community. The FN-CR demonstrates critical thinking and utilizes social work ethics to assess and meet the needs of patients and their families/caregivers by providing care coordination throughout the care continuum using a culturally responsive lens.
Aided by their language skills and cultural knowledge, the FN-CR assists in improving the clinic care experience and preparedness for treatment by orienting and educating patients, families, and caregivers to the healthcare system and multidisciplinary team member roles and resources. The FN-CR will assess and address patients’ barriers to care, partner with patients/families to address their concerns, issues and needs, and facilitate appropriate referrals. The FN-CR will serve as the central point of contact to the patient, facilitating collaboration among all team members and ensuring an individualized approach throughout the length of the patient’s needs. This includes clinical service directly after the initial intake call, during the forensic evaluation and post-evaluation, providing support and engagement with outside resources. The FN-CR will demonstrate commitment to quality patient care, implement creative and innovative ways to meet diverse patient needs, and demonstrate ability to prioritize utilizing organizational skills and clinical judgement. The FN-CR will promote a consistent, holistic plan of care to enhance the quality of life of patients, families and caregivers through assessment, teaching, communication, support, and the practice of family-centered care. The FN-CR will work to influence both positive patient outcomes and system outcomes through improved interdisciplinary communication, patient retention, and community partner collaboration. All services will be delivered in the patient and caregiver’s preferred language.
English and Spanish fluency are essential functions for this position.
Education:
Master’s degree in mental health field, including Social Work, Psychology, Counseling, or Marriage and Family Counseling.
Experience:
Minimum one-year post-MSW experience in one or more of the following areas required: Healthcare, family service, child maltreatment, case management, or coordination of home-based services. Advanced knowledge of and experience working with the Latinx community, as demonstrated by knowledge of traditions, values, and customs of the community and lived or professional experience working directly with individuals who identify as Latinx.
