Overview

The Stroke and Epilepsy Program Coordinator is an exciting dual role providing clinical expertise and leadership within the interdisciplinary teams responsible for the Legacy Good Samaritan Stroke Program and Legacy Health Epilepsy Program. In the role of Legacy Good Samaritan Stroke Coordinator, collaborates with physician leadership and peers both locally and throughout the Legacy system to ensure all program certification standards are being met to optimize the outcomes for stroke patients. In the role of Epilepsy Program Coordinator, engages with patients and epilepsy program team members to ensure smooth coordination of care across the epilepsy services pathway. In both roles, support ongoing nursing education, program development, and initiatives to improve quality of care. Ideal candidates demonstrate strong interest in working with the neurological patient population, have experience in data collection and review, facilitating quality improvement, nursing education, and protocol/standard development and maintenance.

Responsibilities

GENERAL ACCOUNTABILITIES AND ESSENTIAL FUNCTIONS:

 

 

Provides clinical expertise and leadership within the interdisciplinary team and facilitates team coordination in the provision of Stroke & Epilepsy patient care.

Supports the improvement of clinical outcomes through the development, implementation and refinement of best practices/processes centered around evidence-based practice.

Leads or supports implementation and evaluation of initiatives to improve quality of care for the Stroke & Epilepsy patient population.

Tracks and reports disease-specific program data to support overall program management, quality improvement and disease-based program certification (if applicable).

 

Provides patient and community education aimed at improving health and optimizing recovery.

Remains current on the national quality standards and requirements for data reporting and Stroke & Epilepsy care certification as applicable.

Serves as a role model, educator and resource for staff in the care of the Stroke & Epilepsy patient population. Establishes and maintains effective relationships with health care team members across multiple disciplines and departments (i.e., physicians/surgeons, nursing, rehab, pharmacy, imaging, ED, etc.) involved with Stroke & Epilepsy patient care.

 

Engages in continuing education and ongoing professional development to maintain skill and knowledge base in Stroke & Epilepsy care or as required by program certification.

Qualifications

Education:

Academic degree in nursing (BSN or MSN/MN) required. Internal candidates promoting may meet the degree requirement with verification of bachelor’s  or master’s degree in progress with an expected completion date within 2-years of the promotion date; progress will be reported periodically to the site Chief Nursing Officer (CNO).

 

Experience:

2-3 years of experience in the provision and coordination of care for Stroke & Epilepsy patients preferred. Experience leading performance improvement efforts and Stroke & Epilepsy program management strongly preferred.

 

Skills:

Demonstrated skills in patient assessment, multidisciplinary collaboration, problem solving, data analysis, leading change, teaching, coaching, written and verbal communication. Proficiency in Microsoft Office Suite and clinical information systems.
